About Chronic Conditions

According to the National Center for Chronic Disease Prevention and Health Promotion, Six in ten Americans live with at least one chronic disease, like heart disease and stroke, cancer, or diabetes. These and other chronic diseases are the leading causes of death and disability in America, and they are also a leading driver of health care costs.  The Finger Lake Rural Health Network has come together to promote services and tools for patients to help reduce the risk of chronic diseases.

Chronic Conditions Self-Management Program

Chronic Disease Self-Management Program is a six-week program for individuals who suffer from a chronic condition or are a caretaker of someone with a chronic condition. The program will cover techniques to manage symptoms like anxiety, stress, depression, fatigue, pain, frustration, limitations, and more. It will give you tips on how to start eating and exercising to improve your health and how to create an action plan for a healthier life.
